"This editition of the Textbook of Transplantation represents just one of the American Society for Transplantation (AST's) many educational initiatives targeted to the next generation of transplant professionals. In addition to organ-specific chapters (kidney, pancreas, heart, lung and liver), this book includes generic chapters that should be of interest to all readers, irrespective of their organ-specific speciality. These include chapters on immunibiology, pharmacology, donor management,...

Invasive fungal infection (IFI) is a serious threat after allogeneic hematopoietic cell transplant (HCT). This multicenter, randomized, double-blind trial compared fluconazole (N = 295) versus voriconazole (N = 305) for the prevention of IFI in the context of a structured fungal screening program. Patients undergoing myeloablative allogeneic HCT were randomized before HCT to receive study drugs for 100 days, or for 180 days in higher-risk patients. Serum galactomannan was assayed twice...

Summary: Melanized or dematiaceous fungi are associated with a wide variety of infectious syndromes. Many are soil organisms and are generally distributed worldwide, though certain species appear to have restricted geographic ranges. Though they are uncommon causes of disease, melanized fungi have been increasingly recognized as important pathogens, with most reports occurring in the past 20 years. The spectrum of diseases with which they are associated has also broadened and includes...

We report our experience with using integrated molecular and anatomic hybrid imaging to assess infection in patients who have a left-ventricular-assist device (LVAD). Methods: Thirteen 99mTc-exametazime-leukocyte planar and SPECT/CT scans were obtained for 8 consecutive patients who had an implanted LVAD. SPECT/CT was used to assess suspected device-related infections (n = 8) and to evaluate the efficiency of current antibiotic therapy (n = 5). Results: Device-related infection was seen on 8...

Background CSF analysis is often deferred in patients with cryptococcal disease, particularly in the absence of neurologic manifestations. We sought to determine if a subset of SOT recipients with high likelihood of CNS disease could be identified in whom CSF analysis must be performed. Methods Patients comprised a multicenter cohort of SOT recipients with cryptococcosis. Results Of 129 of 146 (88%) SOT recipients with cryptococcosis who underwent CSF analysis, 80 (62%) had CNS disease. In...

This curriculum serves to provide comprehensive guidelines for training fellows in transplant ID.These guidelines will no doubt need to be modi¢ed for the unique environment of each transplant center. Moreover, as the ¢eld changes and new paradigms in transplantation emerge, updates will be required as well.
